Social Work Services

Social work services are a vital component of our home health program. Our goal s to improve or maintain the social, functional and physical health status of each patient while enhancing the coping skills of the family or other caregivers.

Services [may include but are not limited to]:

• Liaison with other member of the health care team
• Advocacy with community agencies
• Information about programs such as Meals on Wheels
• Counseling and crisis intervention and application for financial assistance
• Assistance with long term care planning and placement
• Assistance with Medical Benefits
